• Provide primary nursing care to home‑based patients as ordered by the physician.
• Conduct initial assessment and develop a care plan in collaboration with the Population Health Case Manager.
• Supervise Home Healthcare Aides and LPNs as needed.
• Provide therapeutic interventions and overall care management to achieve top decile client outcomes and patient satisfaction.

• Graduate of an approved nursing education program.
• Registered Nurse license in the state of PA.
• One (1) year professional care nurse experience with home care preferred.
• Valid driver’s license and reliable transportation.
