Job Summary

12 Month Fixed Term - Maternity Cover

An exciting opportunity has arisen for an enthusiastic individual to join our community service. The post focuses on high‑risk frail patients, supporting early detection of medical, social or psychological crises.

The role works with primary care and enhanced community services, carrying a complex caseload. As a new position, you will help develop Frailty pathways across primary and enhanced community services.

As a highly skilled senior AHP practitioner, you will have expert knowledge in frailty management to prevent avoidable hospital admissions and support timely discharge home. You will collaborate with primary, community and secondary care services, providing clinical leadership to teams delivering rehabilitation and developing pathways.

Main Duties

• Conduct autonomous complex assessments and make clinical decisions for patients requiring enhanced community services, including comprehensive geriatric assessment in the home environment.
• Act as a trusted assessor to achieve positive outcomes for patients.
• Focus on preventative strategies, targeting those in the community with complex long‑term conditions and escalating service use.
• Review, assess and support the development of self‑management and social management strategies alongside medical management to reduce ongoing health and care needs.
• Liaise with the multidisciplinary team, including secondary care GPs, patient relatives and third‑sector partners.
• Demonstrate a flexible approach as this is a new post that will evolve with service demands.

Job Responsibilities

Travel between sites in the community is essential. We especially welcome applications from Welsh speakers, but all applicants are encouraged and will receive support for language development if needed.

Person Specification Qualifications & Knowledge (Essential)
  • Diploma/Degree in Occupational Therapy or Physiotherapy
  • HCPC registration
  • Advanced accredited training at master’s level or equivalent
  • Accredited postgraduate specialist courses/training relevant to role
Qualifications & Knowledge (Desirable)
  • MSc in a relevant specialty
  • Relevant leadership courses/qualifications
  • Post‑graduate qualification up to postgraduate diploma level in a relevant area
  • Membership of the Royal College of Occupational Therapy or Chartered Society of Physiotherapy
Experience (Essential)
  • Significant postgraduate experience in the specialty
  • Experience in audit and service evaluation
  • Experience managing a complex caseload within the specialty
  • Experience in multi‑agency team working in a specialist area
  • Leadership of relevant networks or practice groups
  • Evidence of supporting clinical governance, including codes of conduct
  • Research experience
  • Staff development experience, including education and supervision
  • Experience presenting to varied audiences
