Nurse Practitioner / Physician Assistant Primary Care PT

Position Summary:

PT position with 1 to 2 weekdays availability required. Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, medical assistants, and in‑house support staff.

  • Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facilities.
  • Review and sign medication administration records, orders, death certificates, plans of care, PT/OT orders, hospice orders, and other relevant documents in accordance with facility policy and within 24 hours if faxed or emailed.
  • Review and address laboratory results within 24 hours.
  • Manage narcotic prescriptions per facility policy.
  • Complete documentation and submit billing records per published policies; provide hard copy within 48 hours of service.
  • Maintain open communication with facility staff and administration; provide verbal or written instruction regarding medication and caregiver information.
  • Meet with the administrator or designated facility care coordinator prior to visits each day and prior to leaving the facility.
  • Adhere to Eventus Whole Health standards, policies, procedures and all legal and regulatory agency rules including HIPAA confidentiality requirements.
  • Consult with clinical supervisor before refusing a resident, family or facility request or recommendation regarding a resident’s care.
Qualifications / Education
  • Master's degree in Nursing from an accredited institution and a current, valid state practice license issued by the state medical board; all other certifications necessary to perform duties must be current and valid.
  • OR
  • Graduated from an accredited physician assistant program; passed the national certification exam; licensed as a physician assistant in the state of residence; specialization in geriatric, family, or psychiatry desired.
