Eden Brown Synergy are currently recruiting for an experienced Adult Social Worker to join a hospital-based team at Norfolk & Norwich University Hospital.

Travel:
The role is hospital-based and will entail travel.
This is an exciting opportunity for a qualified Adult Social Worker to work within a dedicated hospital team supporting adults during inpatient stays.
You will work closely with health and social care professionals as part of a multi-disciplinary team, helping to facilitate safe and timely hospital discharges and ensuring individuals receive the right support at the right time.
The service follows a Home First ethos, focusing on independence, recovery and supporting individuals to return home wherever appropriate. You will also be involved in reviewing individuals following discharge, either in their own homes or within care settings.
Key Responsibilities- Complete person-centred assessments with adults and their families/carers.
- Develop and review care and support plans.
- Support safe and timely hospital discharge planning.
- Promote independence, wellbeing and positive outcomes.
- Work collaboratively with multi-disciplinary teams.
- Complete Mental Capacity Assessments and contribute to best-interest decisions.
- Support Continuing Healthcare (CHC) reviews where required.
- Review individuals following discharge and ensure appropriate support is in place.
- Maintain accurate case records and documentation.
- Apply the Care Act 2014 and relevant legislation in your practice.
Requirements
The successful candidate will have:
- A recognised Social Work qualification.
- Current Social Work England registration.
- Experience working with adults within social care, hospital or community settings.
- Good knowledge of the Care Act 2014.
- Experience completing Mental Capacity Assessments and best-interest decisions.
- Knowledge and experience of CHC reviews.
- Strong communication, assessment and organisational skills.
- Experience working effectively within a multi-disciplinary team.
- The ability to work independently in a fast-paced environment.
- A commitment to person-centred practice and promoting independence.
- The ability to travel as required.
