About the role

Norfolk is pleased to announce an exciting opportunity for a Senior Social Worker to join our Children with Disabilities (CWD) West and Breckland Team.

Our Children with Disabilities Service supports children and young people aged 0–18 who have substantial and permanent disabilities. The service is delivered through three multi-disciplinary teams covering Norfolk, made up of Social Workers, Family Practitioners, Assistant Practitioners, Occupational Therapists, and Occupational Therapy Assistant Practitioners.

This is a close-knit, supportive team working collaboratively to provide high-quality, child-centered support. The team works with children, young people, and their families from initial referral through their full journey in Children’s Services, including:

  • Children in Need (CIN)
  • Child Protection (CP) Children
  • Looked After (CLA)
  • Transition to Adulthood

We work closely with a wide range of internal and external partners to ensure a holistic and coordinated response to the needs of the children and families we support.

Key Responsibilities
  • Hold and manage a caseload of children and young people with disabilities
  • Complete assessments to identify needs and risks
  • Develop and review tailored support packages to meet identified needs
  • Undertake statutory social work responsibilities across CIN, CP, CLA and transition pathways
  • Work collaboratively with CWD Occupational Therapists and CWD Outreach Services to deliver targeted individual interventions
  • Ensure children and young people are supported to access equal opportunities to their non-disabled peers
  • Maintain high-quality recording and professional standards in line with statutory guidance
