Job Description Sentara Medical Group is seeking a
** Nurse Practitioner or Physician Assistant
** to join its
** Outpatient Palliative Care team
** at the
** Supportive Care Cancer Center
** located on the campus of
** Sentara Norfolk General Hospital in Norfolk, VA.
** The Nurse Practitioner or Physician Assistant will provide clinical care, education, and compassionate support for patients with advanced or serious illnesses as part of a collaborative interdisciplinary team.
** Position Highlights:
*** Monday Friday; 8:00 a.m. 4:30 p.m.
* Performing outpatient palliative care consultations and follow-up visits for patients with complex symptom management and advanced care needs
* Facilitating family meetings focused on medical decision-making, advance care planning, and goals of care discussions
* Collaborating closely with referring oncology providers, social workers, and other interdisciplinary team members to ensure coordinated, holistic, and patient-centered care
* Supporting seamless transitions of care, including coordination with home-based palliative care or hospice programs as appropriate
* Participating in team meetings to promote best practices in symptom management, emotional support, and quality of life improvement for patients and families
