In this role, you will conduct comprehensive in-home health assessments, focusing on adult and geriatric populations, to enhance patient outcomes.

As a provider at Matrix, you will:

• Conduct thorough adult and geriatric assessments, including medical history, diagnosis, treatment, and health education.
• Administer and order necessary point-of-care and lab-based testing.
• Collaborate with healthcare teams, including physicians, case managers, and caregivers.
• Provide services in the comfort of members' homes or Skilled Nursing Facilities.

  Full-time providers typically work Monday to Friday, 8:00 AM to 5:30 PM.
• Competitive Pay: Annual base salary ranging from $95,500 - $158,400 , based on experience.

We’re looking for candidates who possess:

• A Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP/PA licensure with prescription authority.
• Board certification in relevant specialties (NCCPA, AANP, etc.).
• Current BLS, ACLS, or CPR certification.
• 1 year of NP or PA experience preferred; new grads are encouraged to apply!
• A valid driver’s license and proof of insurance.
• Strong communication skills and computer proficiency.
