City of Norfolk seeks a Case Manager II in the Norfolk CSB to provide linked services for adults with serious mental illness and co-occurring disorders. You will assess needs, develop and monitor ISP plans, and coordinate with providers to secure housing, health care, and other supports.
Typical hours are Monday–Friday 8:30 AM–5:00 PM. The Departmental Hiring Salary Range is $49,152–$56,403.
