Overview

The Village at Mercy Creek, part of Franciscan Ministries, is a vibrant senior living community in Normal, IL. We offer Assisted Living and Memory Care and are looking for Part‑Time RN/LPNs for 2nd shift. Sign‑On Bonus: $2,500 and shift differential.

Position Summary

Provide direct nursing care for residents and general consultation regarding health‑related concerns within the community.

Essential Functions, Key Duties & Responsibilities

  • Direct the daily activities of certified nursing assistants and resident care associates in accordance with regulations and company policies.
  • Follow the nursing model to promote holistic care.
  • Ensure policies and procedures are complied with by nursing personnel.
  • Participate in admission, discharge and transfers of residents as required.
  • Assess changes in residents’ status, notify physician and family, and document accordingly.
  • Transcribe physician orders to the medical record and carry them out.
  • Prepare and administer medications per physician orders and observe for adverse effects.
  • Perform wound treatments per physician orders and document changes.
  • Schedule follow‑up appointments and transportation needs as indicated.
  • Provide nursing leadership to personnel assigned to the unit.
  • Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. per policies.
  • Perform rounds to ensure resident needs are met and duties are performed.
  • Collaborate with interdisciplinary team to ensure holistic care.
  • Report incidents or unusual occurrences to leadership.
  • Maintain adequate stock of medications, supplies, equipment; notify needs.
  • Initiate, review and update care plans as required.
  • Participate in survey process when inspections arise.
  • May participate in QAPI activities as assigned.
  • Ensure infection control principles are followed.
  • Perform other duties as assigned.
  • Comply with all policies and updates.

Position Requirements

Education
:
Nursing degree from an accredited college or university.

Licenses/Certifications
:
Current unrestricted LPN license in the practicing state.

Experience
: 1+ years of nursing experience (preferred).
