Empower Me Wellness is seeking therapy professionals to join our Empower Me 360 program, integrating therapy, wellness, and medical care for seniors in our partner communities. You will guide residents to live stronger, safer, and more independent lives through physical, occupational, or speech therapy.
You’ll collaborate with a multidisciplinary team, perform assessments, build personalized plans, and participate in care coordination, telehealth support, and quality improvement efforts.
