Wound Care Nurse (RN) or LVN
Medical Park West Rehabilitation and Skilled Care
, Norman, OK has an opportunity for a Wound Care Nurse (RN) or (LVN) to join our team! The role will promote, coordinate, and monitor our state‑of‑the‑art infection control program within assigned skilled nursing and assisted living facilities. Under the leadership of our Chief Clinical Officer and Director of Infection Control Programming, the Regional IC Consultant will work directly with regional nurse consultants, directors of operations, and facility administrators to ensure full implementation of the program in compliance with CDC, CMS, OSHA and other guidelines.
- Investigate, monitor, and communicate known or potential sources of suspected infections to determine sources and minimize spread.
- Demonstrate strong knowledge of standard methods and criteria for identifying nosocomial infections using an epidemiological approach.
- Instruct and counsel patients and staff on infection control issues and concerns.
- Collaborate with team members to follow up on exposure incidents.
- Serve on the Quality Committee.
- Provide infection control monthly summaries and alerts to facility leadership and the Quality Committee.
- Utilize computerized documentation systems to access relevant patient data.
- Provide new staff orientation and ongoing mentoring regarding infection control practices.
- Follow standard precautions and appropriate CDC‑recommended isolation categories using PPE as required.
- Ensure processes are in place to identify and monitor patients requiring isolation for an infectious disease.
- Implement the infection control policy manual.
- Provide ongoing education to nursing and ancillary staff on infection control practice and regulatory standards.
- Coordinate annual review, revision, and evaluation of the infection control plan.
- Participate in staff meetings, patient care conferences, and other committees as required.
- Maintain ongoing education to improve job performance.
- Demonstrate sensitivity to culturally diverse patient populations.
- Proactively seek team building and positive working relationships with all staff.
- Participate in performance improvement activities and seek opportunities to improve inter‑department processes.
- Demonstrate flexibility regarding assignments.
- Perform special assignments as needed or requested.
Position Requirements
- Current RN or LVN license in the State of Texas.
- Good interpersonal, verbal, and written communication skills.
- Ability to work independently and prioritize responsibilities.
