Social Work Case Manager job at State of OK JD McCarty Center. Norman, OK. As a Social Work Case Manager, you will play a pivotal role in providing comprehensive social services and support to individuals and families in need. This position requires a deep commitment to our patients including empathy and a proactive approach to problem-solving. You will work closely with patients and families to assess their needs, develop care plans, coordinate resources, and advocate for their well-being.

Salary starting at: $50,000 with salary based on education and experience.
Key ResponsibilitiesField intake calls and connect families and/or caregivers with appropriate referrals. Conduct thorough assessments of patient’s social, emotional, financial, and environmental needs. Provide crisis intervention and emotional support to patients and their families as needed. Develop individualized care plans in collaboration with patients, families and multi-disciplinary treatment teams, identifying goals and interventions to address their specific needs. Communicate patient progress by facilitating and/or attending interdisciplinary meetings, communicating case status with the treatment team, and identifying barriers to treatment success.
Act as liaison between the patient’s interdisciplinary treatment team, parent/guardian, and external service providers for patient care needs. Collaborate effectively with interdisciplinary teams including healthcare professionals, social workers, psychologists, and community agencies. Aid in the admission identification process including reviewing and reporting on prospective inpatients. Assist with scheduling and coordination of inpatient admissions as assigned. Coordinate and connect patients with appropriate support services that are available at J.D. McCarty Center and throughout the state, such as outpatient therapies, Developmental Disability Services, childcare programs, Supplemental Security Income (SSI), and other local and statewide resources.
Advocate for patient’s rights and access to services within various systems including healthcare, education, and social services. Monitor patient’s progress towards goals, regularly reassessing needs and adjusting care plans as necessary. Work closely with the Oklahoma Department of Human Services (including Child Welfare, Child Protective Services, and Developmental Disability Services) with the care of children in state custody and aiding in the facilitation of transition planning for placement opportunities.
Maintain accurate and confidential client records, documentation, and reports within the time frame and guidelines specified by hospital and departmental policy. Prepare client discharges by reviewing and altering discharge plans as needed, coordinating discharge requirements, outlining goals achieved, and providing resources. Facilitate transition planning for patients upon discharge. Assess training needs for families, caregivers, schools, or other service providers related to understating the patient’s treatment plan and coordinates any applicable training with internal and external entities.
Serves on various agency, community, and state committees which are directed towards the achievement of department and agency goals, mission, and vision. May assist with maintaining and assessing QA measures for department. Completes other duties as assigned.
- Bachelor's degree in Social Work (BSW) from an accredited program.
- Proven experience in case management, social work, or related field preferred.
- Strong knowledge of social services, community resources, and public assistance programs.
- Excellent interpersonal skills with the ability to build trust and rapport with diverse clients.
