Integration Specialist (Direct Care)

Schedule:

Monday‑Friday; flexible hours.

Pay Range: $20.00 – $22.00 per hour.

Responsibilities
  • Develop a deep understanding of each person supported, including their strengths, needs, goals, and desires.
  • Assist individuals in articulating their needs, goals, and desires to facilitate personalized care.
  • Work collaboratively as a member of a team to support individuals in developing a full community‑based daytime life.
  • Collaborate with Program Director, Assistant Program Directors, Shared Living Providers, and other stakeholders.
  • Support and exert personal initiative to ensure each person presents a positive image of themselves and the program.
  • Assist individuals in developing long‑lasting and meaningful relationships with family, friends, community members, co‑workers (where applicable), and becoming active community participants.
  • Assist individuals in participating fully in community activities, including group memberships, volunteer commitments, educational opportunities, leisure and social pursuits, and employment where applicable.
  • Complete Individual Service Plan goals and activities in collaboration with the team.
  • Monitor activities of daily living (ADL) and provide necessary support.
  • Work closely with guardians, family members, community health care practitioners, and other institutions to ensure comprehensive and coordinated care.
Qualifications
  • 2 years of experience working with individuals with developmental disabilities (preferred).
  • High School Diploma.
  • Computer and organizational planning skills.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a team environment.
  • Flexibility and adaptability to meet changing needs.
  • Community experience, especially with Department of Developmental Services, Adult Foster Care Services, HCBS waivers (supporting those with brain injury or chronic conditions).
  • Background check required.
  • Valid driver’s license with an excellent RMV record (subject to background check).
