This is a full-time position with 35-hour work week and flexible/hybrid schedule.

Hope For Youth has an exciting and rare Therapist position opening in its PINS Diversion program. The Nassau County PINS (Person In Need of Supervision) Diversion program is designed to divert youth who are at risk, from PINS petitions that require court involvement. PINS Diversion services families with children up to the age of 18 in Nassau County. Families that apply for diversion services have issues including but not limited to school truancy, gang involvement, runaway behaviors, mental health and substance abuse issues.

The PINS diversion program provides short term intensive in-home, family-centered interventions with an overarching goal that will prevent out of home placements, teach problem solving skills to the families, and linkage to a community-based system of care.

A unique component to the diversion program is the incorporation of Multidimensional Family Therapy (MDFT). MDFT is an evidence based program designed to meet the needs of youth struggling with mental health and substance abuse issues. Paid training in this model is provided to the hired Therapist during normal work hours at no cost.

Key Responsibilities
  • MDFT Therapists function within Agency's policies and procedures as outlined in the Employee Manual.
  • MDFT Therapists function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
  • MDFT Therapists provide support to the clients and families-i.e., the knowledge, skills and tools necessary in order for them to maintain the child in their home.
  • MDFT Therapists achieve successful outcomes within the guidelines outlined in NYS, County and HFY policy and procedure (shared).
  • MDFT Therapists perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations.
How MDFT Therapists will generally spend time in this job
  • In accordance with agency policy and procedures, the MDFT Therapists will be a Licensed Therapist (LMSW, LCSW, LMHC, LMFT) and will develop a comprehensive clinical treatment plan that includes counseling, mental health and substance abuse treatment services for youth with a High acuity level for behavior and trauma as determined by the CANS-NY. The MDFT Therapists will:
  • MDFT Therapists provide individual clinical services to a caseload of up to 12 clients at any time and the program will service up to 50 clinical cases total annually.
  • The MDFT Therapist training program will be conducted within a 3- to 6-month time frame. A full course can range between 30 and 60 sessions over 6 months, with clients seen 1 to 4 times a week as needed.
  • MDFT Therapists will administer a mixture of home, community or office-based interventions.
  • MDFT Therapists conduct collateral contacts as appropriate in school or community-based locations.
  • MDFT Therapists prepare psychosocial assessments, treatment plans, progress notes to document all contacts, interdisciplinary team conference documentation and other such necessary case related information.
  • Primary physical requirements of MDFT Therapists: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, ongoing access to reliable vehicle and driving own vehicle for work. A vehicle inspection may be conducted during the interview.
Qualifications
  • License Required (LMSW, LCSW, LMHC, LMFT)
  • Spanish and English-speaking preferred
  • Experience is preferred, and Hope For Youth will train in the MDFT model.
  • Valid NYS Driver's License in good standing.
