Job Title: Nurse Practitioner - Adult Geriatric
Job DescriptionUnder the supervision of the Chief Medical Officer, this position functions in a team and mission-based setting to provide primary and specialty healthcare to adult and geriatric patients. This includes health promotion, disease prevention, health education, and counseling. This position exercises progressive levels of autonomy in day-to-day activities based on satisfactory job performance.
Job Duties /Responsibilities- Adhere to the spirit of Waterfall Community Health Center and our Mission Statement to remove barriers to care while performing assigned duties.
- Demonstrate the core values of Integrity, Respect, Accountability, Compassion, Partnership, and Collaborative Communication.
- Obtain and record client health history; perform comprehensive physical assessments (including biological, social, and psychological needs) as part of a total physical examination.
- Demonstrate knowledge of interviewing and counseling techniques.
- Diagnose, treat, and plan appropriate health care for adults in primary and specialty health care as appropriate for the specialty; refer clients to physicians and/or outside agencies as required.
- Demonstrate skill in nursing/medical techniques and procedures.
- Provide and prescribe medications within the area of specialty.
- Perform laboratory procedures and interpret lab test results.
- Provide group, individual, and community education classes on health concerns for an adult population.
- Establish and maintain client medical charts and other necessary records.
- Provide in-service education classes on assigned responsibilities for clinical staff.
- Demonstrate skill in diagnostic procedures and drug therapy needed for treatment.
- Proficient in the use of medical and laboratory equipment.
- Participate in Clinical Quality Improvement and Quality Management meetings and other meetings as requested by the supervisor and/or Senior Leadership Team.
- Participate in ongoing clinical practice evaluation of other clinicians (Peer Review).
- Demonstrate ability to work independently, communicate effectively verbally and in writing, and maintain effective working relationships with related agencies, professionals, and citizen groups.
- Promote integrity by exhibiting ethical behavior to maintain high standards.
- Maintain professional affiliations and development to stay current with professional literature and health care management trends and regulations.
- Perform any other duties needed to help drive the Organization’s Vision and Mission and uphold its Values.
- 3 years’ experience preferred.
- Unrestricted License in the State of Oregon.
- Current DEA Certificate without restrictions.
- English language proficiency required; additional languages preferred.
- Ability to be credentialed with Medicaid, Medicare, private, and other insurance.
- Demonstrated commitment to high-quality health care for low-income, ethnically diverse populations.
- Licensed Nurse Practitioner;
Adult/Geriatric Board Certified or Eligible;
Fully Credentialed and Insurable. - Current American Heart Association (AHA) Provider Basic Life Support (BLS) certification required within 2 months of hire. Maintenance of certification required.
- Non-violent Crisis Intervention:
Comprehensive training completed within 6 months of hire and recertified prior to expiration date.
- TB test (upon hire).
- Prolonged periods of sitting, bending, and/or leaning over patients, or standing at a computer.
- Repetitive use of hands and handheld tools.
- Able to lift up to 20 pounds. Able to travel as needed.
