Job Description

Job Purpose: Under the supervision of the Chief Medical Officer, this position functions in a team and mission-based setting to provide primary and specialty healthcare to adult and geriatric patients. This includes health promotion, disease prevention, health education, and counseling. This position exercises progressive levels of autonomy in day-to-day activities based on satisfactory job performance.

Job Duties / Responsibilities
  • Adhere to the spirit of Waterfall Community Health Center and our Mission Statement to remove barriers to care while performing assigned duties.

  • Obtains and records client’s health history; performs comprehensive physical assessment (including biological, social, and psychological needs) as part of a total physical examination.
  • Demonstrate substantial knowledge of interviewing and counseling techniques.
  • Diagnoses, treats, and plans appropriate health care for clients in the area of adult primary and specialty health care as appropriate for the specialty; refers clients to physicians and/or outside agencies as required.
  • Demonstrate skillful knowledge of nursing/medical techniques and procedures.
  • Provides and prescribes medications within the area of specialty.
  • Performs laboratory procedures and interprets lab test results.
  • Ability to provide group, individual, and community education classes on health concerns for an adult population.
  • Establishes and maintains client medical charts and other necessary records.
  • Provides in-service education classes on assigned responsibilities for clinical staff.
  • Demonstrate skill in diagnostic procedures for diseases and the drug therapy needed for treatment.
  • Proficient in the use of medical and laboratory equipment.
  • Participate in the Clinical Quality Improvement and Quality Management meetings and other meetings as requested by the supervisor and/or the Senior Leadership Team.
  • Participates in ongoing clinical practice evaluation of other clinicians (Peer Review).
  • Demonstrate ability to work independently, communicate effectively both verbally and in writing, deal tactfully and patiently with clients, maintain effective working relationships with other related agencies, professionals, and citizen groups.
  • Promote individual and organizational integrity by exhibiting ethical behavior to maintain high standards.
  • Maintain professional affiliations and enhance professional development to remain abreast with professional literature and current health care management trends and rules in FQCH regulation.
  • Performs any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values.
Education, Experience, and Qualifications
  • 3 years’ experience preferred.
  • Unrestricted License in the State of Oregon.
  • Current DEA Certificate without restrictions.
  • Language

    Skills:

    Able to communicate effectively in English, both verbally and in writing; additional languages preferred.
  • Ability to be credentialed with Medicaid, Medicare, private, and other insurance.
  • Demonstrated commitment to high-quality health care for low-income, ethnically diverse populations.
Licensure / Certifications
  • Licensed Nurse Practitioner;
    Adult/Geriatric Board Certified or Eligible;
    Fully Credentialed and Insurable.
  • Current American Heart Association (AHA) Provider Basic Life Support Card (BLS) certification required within 2 months of hire. Maintenance of certification is required.
  • Non-violent Crisis Intervention:
    Comprehensive training class completed within 6 months of hire and recertified prior to the expiration date.
Immunizations Required
  • TB test (upon hire).
Physical Requirements
  • Prolonged periods of sitting, bending, and/or leaning over patients, or standing at the workstation using a computer.
  • Repetitive use of hands and handheld tools of the trade.
  • Able to lift up to 20 pounds at times.
  • Able to travel as needed.
