• Serve as the attending physician for residents in each assigned SNF.
• Collaborate with onsite NPs/PAs to manage daily patient care and provide clinical supervision.
• Provide high‑quality, patient‑centered primary care, diagnosing and treating acute and chronic conditions.
• Complete timely documentation in Chart Easy™ and work with facility EHRs (Point Click Care , Matrix Care, AHT).
• Participate in Theoria’s Chronic Care Management, Transitional Care, RPM, and other quality initiatives.
• Utilize electronic medical systems, including EHRs and state/county tools for clinical and administrative tasks.
