The Hopscotch Care Team is responsible for delivering high-touch, high-quality care to patients, including coordination of care across different physicians and the broader healthcare continuum.
What You'll Do
As the Medical Director, you will provide day-to-day leadership for the clinic teams and deliver high-quality care, an exceptional patient experience and a best-in-class culture, acting as a Champion of the Hopscotch Care Model and a Culture advocate for your team and your patients. Specifically, you will:
Provide leadership, oversight and support for the team to drive day-to-day execution against critical priorities to enable high-quality care, a great patient experience, engagement across the team and a best-in-class cultureDefine objective goals and top priorities for the clinic and for each member of the team, in partnership with Hopscotch central team members, to support clinical, growth, and cultureDefine team and culture priorities, in collaboration with Care Center Manager, ensure time/resources are invested to build and maintain a great culture and drive engagement of the teamChampion the Hopscotch Care Model and brand in the community, sharing the mission and spreading the word to enable more Medicare patients to receive this enhanced level of careEnsure operational needs are proactively and collaboratively managed with the Care Center Manager, including provider coverage to ensure patient care is continuousSet the curve for Care team and across Care Centers through clear understanding and execution to deliver care in the Hopscotch care model, a commitment to results, quality and excellence and by executing against key workflows and processes to enable the modelManagement of Providers including ensuring all Providers complete administrative tasks and utilize indirect care hours appropriately, managing their panels and completing documentationLead by example and drive execution across teams by role modeling workflow efficiencies including accurate and timely manner for preparation, delivery and follow through of patient visits, including documentation and codingActively participate in all Clinical Forums, providing feedback, driving action and ensuring that key discussion points and takeaways are cascaded to your clinic Providers and other team members as appropriateCoaching and Mentoring of Providers in order to support integration, transformation, and on-going operational excellence at the clinic, including but not limited to comprehensive care model training, migration to athena EMR, renovations of the clinic, onboarding of new care teams roles, and reassignment of roles for the existing team membersPartner with the Care Center Manager to outperform against target metrics and outcomes, including patient…
