Job Responsibilities- Provide professional social work services to clients in diverse settings (human service agencies, correctional facilities, teaching hospitals, medical schools, psychiatric hospitals, and mental retardation centers).
- Evaluate clients' situations, develop social histories, psychosocial assessments, service plans and/or treatment plans, and conduct follow-up.
- Offer basic counseling, support, and reinforcement in areas such as death and dying, employment, vocational, medical, health, or behavioral issues.
- Address problems related to adjustment to illness or disability, placement into an institution, rest home or nursing facility, financial concerns, and socialization issues for the socially handicapped or those debilitated by age or illness.
- Provide information and confer with schools, natural parents and foster parents on habilitation plans, behavior management, and other problem areas or needs.
- Recruit, evaluate, and train foster parents and other care providers; develop community or service resources and/or volunteer programs.
- Participate in habilitation/treatment teams in certain settings and provide the social work perspective.
- Perform assessments to identify needs, establish an appropriate service/treatment plan, and provide supportive/directive counseling to clients and families.
- Serve as inter/multi‑disciplinary team members and refer clients to a variety of services to meet financial, psychological, emergency care, emotional, or other needs.
- Assume a case manager role to follow the client through agency and external services.
- Document direct client interactions as per agency requirements; review through quality control, technical and administrative review.
- Provide direction or instruction to lower‑level staff, volunteers or care providers; decisions impact clients, families, and facility operations.
- Collaborate with clients, families, habilitation/treatment teams in the development and implementation of treatment plans; coordinate services and provide consultation to the community.
- Work in various settings ranging from modern public service buildings to correctional facilities, community residential facilities, home environments, or institutions; address hazards such as emotional stress and potential bodily injury from aggressive clients.
- Demonstrate knowledge of social work principles, techniques, and practices and their application to casework, group work, and community problems; knowledge of psychosocial, socioeconomic, and behavioral problems and their treatment; knowledge of governmental, private organizations, and community resources; knowledge of laws, regulations, and policies; general knowledge of medical terminology, disease processes and their treatment.
- Report to a higher‑level professional or program manager; receive new program requirements/regulations and on‑the‑job training from supervisors.
