School Based Health Patient Support Representative

We are opening a school health center in Graham County Schools this fall. Appalachian Mountain Health is seeking a motivated and caring Patient Support Representative to support healthcare services provided to students and staff at our school health center serving Robbinsville Elementary, Middle, and High Schools in Graham County, NC.

Description
  • Answer screening prioritizing and delegating large amounts of telephone inquiries in a friendly professional manner including de‑escalation skills for disgruntled patients.
  • Return patient, provider and third party calls in a timely manner.
  • Refer patient inquiries of a clinical nature to the appropriate clinical staff for triage.
  • Schedule appointments in accordance with AMH guidelines relating to new patient, routine follow up and same‑day appointments.
  • Provide a welcoming first impression to the organization.
  • Patient check in and complete full registration process, including consent forms.
  • Experience with verifying patient insurance is preferred but not required.
Qualifications
  • Sending and following up on messages between patients and providers/clinical staff via EHR.
  • Read through previous messages/encounters related to patient inquiry to communicate with patient and staff efficiently.
  • Maintain electronic health records (EHR) and coordinate patient activities.
  • Verify information on file and update as necessary.
  • Use data and patient registries to outreach to patients with preventive care and follow up for chronic conditions.
  • Utilize working knowledge of up‑to‑date insurance credentialing information to appropriately schedule appointments.
  • Triage phone calls and follow up on patient requests.
Skills
  • Exceptional written and verbal communication skills.
  • Proficient with medical office technology and equipment, including computers, copiers, scanners, fax machines, and phone systems, as well as MS Excel/Word/Outlook, Electronic Health Record systems and automated medical scheduling and billing programs.
  • Excellent interpersonal skills in group and community settings.
  • Ability to plan, schedule and coordinate.
  • Effective management of multiple duties and responsibilities in a clinical setting.
  • Sound leadership and clinical judgment.
  • Strong attention to detail.
  • Exceptional customer service skills.
Education / Experience
  • High school diploma or General Educational Development (GED).
  • Knowledge of HIPAA Requirements and Guidelines.
