Home Health for a Home Health contract assignment.
Job Title: Physical Therapist:

Requirements:
- [YEARS] years of experience as a PT
- Qualified applicants MUST have at least [YEARS] years of experience in Home Health
- Valid Physical Therapist license
- Be willing to obtain [STATE] licensure
