Tailored Care Manager – Join our multidisciplinary Tailored Care Management team serving Bertie and Chowan counties and surrounding areas. We are dedicated to improving the lives, health, and well‑being of individuals and families impacted by substance use and mental health challenges. Join Our Mission to Make a DifferenceAs a Tailored Care Manager, you’ll evaluate clients’ needs, develop personalized care plans, and coordinate services that address physical and mental health, social determinants of health, and other life challenges.
You’ll travel throughout Bertie and Chowan counties, and surrounding counties, to close gaps in care and improve community lives every day.
Why Join Us?Competitive salary: $53,000 – $55,000 annually.
Full‑time, Monday – Friday schedule (8 AM – 5 PM) with rotating on‑call responsibilities.

Compassionate, driven professionals who are passionate about helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after‑hours support.
Required Qualifications- Bachelor’s degree in health, psychology, sociology, social work, nursing, or a related human services field (Master’s preferred).
- Eligibility for Qualified Mental Health Professional (QMHP) credentialing.
- Minimum two years of experience with mental health and/or substance use populations.
- Proficiency in Microsoft Office and Electronic Health Records (EHR).
- Valid driver’s license, current auto insurance, and a clean driving record.
- Strong communication skills and ability to work independently.
