Foothills (Ellenboro, NC) at Red Oak Recovery is a clinically dynamic, trauma‑intensive substance abuse and adult men’s mental health treatment program designed specifically to meet the unique needs of our clients. We are seeking a Discharge Planner to provide case management to clients with substance use and co‑occurring mental health disorders.

Responsibilities- Provide holistic and comprehensive case management services to all clients including intake assessment, biopsychosocial assessment, ASAM, utilization reviews, goal setting, and weekly progress monitoring.
- Act as an initial contact for each new client.
- Ensure timely completion of client orientation pack.
- Ensure client has resources and knowledge to progress through the program and towards resolution of treatment plan goals.
- Assist with vacation coverage of caseloads and groups for Primary Therapists as needed.
- Facilitate psycho educational groups as directed and attend clinical groups as needed.
- Assist guides and therapists as needed.
- Maintain an active case‑management caseload, providing interventions as needed and within area of expertise and limits of credentials.
- Work with the client on a day‑to‑day basis assessing client needs and acting as a client advocate.
- Provide crisis management for clients; make linkages for interventions as appropriate.
- Maintain client and program records in accordance with applicable standards and regulations of Licensure and Accreditation Boards.
- Maintain and update client release of information to third parties.
- Provide aftercare calls for clients as directed by the Primary Therapist.
- Meet each client weekly for a one‑on‑one assessment and document meeting in EMR.
- Attend clinical meetings and meet regularly with supervisor to exchange pertinent information and receive supervision.
- Work in collaboration with other professionals as part of a treatment team to ensure optimum client outcomes.
- Develop and maintain relationships with referring professionals.
- Act as client liaison regarding medical needs and collaborate with the medical department/professionals in implementing treatment plans.
- Assist Primary Therapist with aftercare needs and connecting with legal authorities as directed by the Primary Therapist.
- Manage client graduation/discharge process.
- CADC or CADC‑I.
- Excellent written and oral communication skills.
- LCMHC‑A or LCSW‑A.
