Four Seasons Compassion for Life is seeking a Social Worker to deliver psychosocial support to hospice patients and families. You will assess needs, develop and implement care plans, and liaise with community resources to coordinate comprehensive care.
Responsibilities include discharge planning, family counseling, and facilitating referrals. Requires a Social Work degree (BSW/MSW) and 1–3 years healthcare experience; valid driver’s license is preferred.
