This req is to provide PT services to Vance and Warren Counties.

How many miles per week will the consultant travel: 30/70 a day

What are the weekly units of productivity expectations: 30 points

If so how much:
No on call but my PT's rotate working a Saturday a month for SOC's.

Do you want the candidate to have home health experience: YES Home visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.

Average # of patients/day: 5-6

Points per day/week:
How are points broken down: 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.

No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.

Varies per location and profession please have traveler discuss during the interview.

Territory/County covering:
Must be willing to travel up to 75 miles from home clinic. Mileage reimbursed IRS Standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable.

Yes local accepted at the same rate.

Pending License accepted: (Y/N) No must have license in hand unless license takes a few days to obtain. Then okay to submit.

Some positions specifically in the western territory often have a Baylor Shift schedule: 4x10s Friday-Monday. Most will be standard 5x8s M-F.
