Schedule:

Traveler Requirements:
- Current license in hand; recent adult home health experience; CPR/BLS certification; reliable transportation and agency-required liability insurance
Responsibilities:
- Provide physical therapy services to patients in their homes and care facilities.
- Conduct start of care visits, follow-ups, and other home health activities.
- Document patient assessments and treatment plans using electronic medical records such as Home Care, Home Base, Oasis, and Outlook.
- Travel up to 75 miles from the home clinic to serve Vance and Warren Counties.
- Rotate working a Saturday per month for social on-call needs.
