Summary:

The Licensed Practical Nurse maintains accountability for the delivery of safe and competent nursing care to assigned patients. Provides care by using the nursing process under the direction and supervision of a Registered Nurse. Supports and assists the Registered Nurse in provision of care and collaborates with physicians and multidisciplinary team members to achieve the highest standards of care specificity to the age of the patient.

Responsibilities

Utilizes and records the nursing process as delegated by the Registered Nurse.

Collects and documents selected data.
Identifies patients’ needs.
Evaluates health related data, including psychosocial information, and participates in identifying solutions.
Collects data in a systematic ongoing process.
Answers patient call lights promptly.
Accurately maintains patients’ charts.
Detects changes in patient’s condition.
Measures and records patient vital signs.

Participates in the promotion, maintenance, and restoration of health, provides comfort measures for a dignified death.

Practices clinically competent and caring behavior.
Assists with preparation of plan of care with patient/family/significant other.
Assists in identifying changes and trends in patient’s status.
Participates in modifying nursing interventions based on changes in patient’s status.
Administers prescribed medications or intravenous fluids.
Collects samples such as stool, urine and sputum from patients.
Administers treatments and special procedures as directed by Registered Nurse or Physician.
- Responds to patient requests for comfort measures promptly. Nursing care includes pain management, medication administration, nutrition, skin and airway integrity, elimination and mobility. Problems and changes in patient’s condition are reported to Charge Nurse/MD as they occur. Interventions and patient responses are documented and reported throughout the shift. Patient is able to verbalize plan of care. All orders are completed for the shift.
- - Completes medication and treatment passes in a timely manner and in accordance with hospital policy.
  - Specimens are collected according to Standards.

Implements care that is legal, ethical and respectful of patient’s rights.

Respects and maintains patient’s right to privacy and confidentiality.
Seeks assistance of Registered Nurses and available resources to help formulate ethical decisions.
Delivers care in a manner that preserves and protects patient autonomy, dignity and rights.
Expresses sensitivity to patient’s culture/religious diversity.
- Patient information is communicated only to those identified as aiding in care. Patients are informed of their rights and their questions are answered. Charge Nurse is informed during shift of ethical concerns patient expresses, and guidelines are followed as instructed. Care is provided to each patient in a nonjudgmental and nondiscriminatory manner.
- - Maintains confidentiality of all patients information.
  - Ensures that all nursing care is provided in privacy.
  - Reviews complaints and grievances made by patients.

Provides a safe, secure patient environment.

Follows Safety and Infection Control Principles.
Completes treatments, medications, and IV administration following defined policies and procedures.
Provides patient comfort and hygiene measures.
Participates in plan of care based on age/population specifications.
- Universal precautions and isolation procedures are followed. Hazardous waste is properly disposed. Safety precautions are implemented. Medication and IV calculations are accurate. Nursing care is provided in accordance to age and corresponding stages of growth and development.
- Follows proper hygiene and comfort procedures for patients and their environment.

Collaborates with other health care team members in providing care.

Maintains accountability for duties accepted, including care given by self and by others to whom care is delegated.
Seeks and utilizes appropriate referral processes.
