At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. We’re seeking a Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team
—a collaborative, multidisciplinary team providing life-changing care to individuals living with serious mental illness and substance use challenges in their own communities.
Your Role in Our Mission
As a Registered Nurse on our ACT Team, you’ll provide direct, compassionate, and holistic care to clients where they live and thrive. You’ll be part of a supportive team that meets daily, shares insights, and works together to create lasting stability for each individual served.
You Will
- Deliver essential nursing care, including psychotropic medication administration and monitoring as prescribed by the team psychiatrist.
- Conduct nursing assessments and interventions while monitoring for medical issues and potential side effects.
- Collaborate with the team psychiatrist to create, update, and manage medication protocols, policies, and procedures.
- Assess and document medication effectiveness, potential complications, and any side effects.
- Develop personalized strategies to promote medication adherence and overall wellness.
- Contribute to individualized treatment plans and participate in daily ACT team meetings.
- Provide on-call crisis intervention services as part of a rotating schedule.
- Document all client interactions and care in the Welligent EMR system.

Compensation & Benefits
- Salary: $70,000 – $85,000 annually (based on experience)

What We’re Looking For
- Active, unrestricted RN license to practice in North Carolina
- Experience in behavioral health or mental health preferred
- Comfort working in community-based settings
- Valid driver’s license and reliable transportation required
