• Performs billable services such as Annual Wellness Visits.
• Functions as liaison between patients, physicians/providers, and staff to ensure accurate communication.
• Conducts Nurse Visits per protocol.
• Performs Medicare Annual Wellness Visits, including measurements of height, weight, BMI, BP, and other routine metrics.
• Reviews Health Risk Assessments (HRA) and addresses patient concerns.

• Associate’s Degree in Nursing (required)
• Bachelor’s Degree in Nursing (preferred)
• Basic Life Support – CPR Certification (BLS)
• Registered Nurse (RN) certification; RQI Resuscitation Quality Improvement
• Ev Accredited Issuing Body certification
