Wound Care Treatment Nurse – YAD Healthcare
Hertford Rehabilitation and Healthcare Center, located in Hertford, NC, is a Long‑Term Care facility that provides quality care to our residents. We are hiring a Full‑Time Wound Care Nurse who will play a critical role in delivering superior nursing care and skin treatments to all residents.
Summary / ObjectiveIn keeping with our organization’s goal of improving the lives of the Residents we serve, the Wound/Treatment Nurse plays a critical role in providing superior customer service and nursing care to all Residents. The Wound Nurse is responsible for performing skin treatments for all Residents under their care. This position will obtain treatment orders from attending physicians and assist with modifying the treatment regimen in accordance with established policies and procedures.
The Treatment Nurse will provide supervision of staff and will safeguard the health, safety and welfare of all Guests under their care by following applicable laws, regulations, and established nursing policies and procedures.

Daily- Complete a Head‑to‑Toe body audit on all new admissions and re‑admissions to ensure that all open areas have been identified by the admitting nurse. Ensure skin sheets, treatment orders, and preventative measures (T&P, specialty mattress, heels up or boots) are in place.
- Review residents who have been identified to have new skin alterations and ensure implementation of skin sheet, treatment, specialty mattress, Braden scale updates, T&P orders, heels up orders or boots (if applicable), and initiate a care plan.
- Update skin sheets every seven days; choose a particular day and ensure updates are completed by the close of business on that day.
- Unit managers update care plans weekly for pressure and non‑pressure areas; care plans must be updated every week by Friday.
- Submit a wound report to the VP of Clinical Services by Thursday reporting the percentage of Center‑Acquired Pressure Ulcers. Our goal is 3% or less.
- Treatments for vascular, surgical, and pressure wounds of stage 2 or greater are completed by the Wound Nurse Monday‑Friday.
- Skin tears of stage 1 are treated by the Charge Nurse Monday‑Friday with completion of the skin sheets weekly. The Unit Manager and Wound Nurse must ensure these skin sheets are being completed by the charge nurse weekly. Unit Managers remind Charge Nurses of the weekly responsibility each day and check the following day for completeness.
- Prevent wounds: provide daily education to the staff, reminding T&P residents (in wheelchairs, Geri‑chairs, and in bed), toileting, and incontinent care. A proactive vs. reactive approach to wound care will keep the facility successful in wound care management. Nurses and CNAs are held accountable for preventative maintenance. It is a team effort to heal wounds and to not create new wounds.
- When wounds worsen, treatment is changed within a two‑week period with input from the wound physician and PCP at the facility.
- Implement preventative items: orders for T&P, preventive barrier cream for all residents (using a barrier cream with Zinc), pressure‑relieving devices for chairs. If a resident has a stage 4 they should be on a Roho or comparable. Every resident should have a gel cushion at minimum for their wheelchair/Geri‑chair. Braden assessments are completed with all new admissions and re‑admissions, quarterly, and with any change in condition.
If the Braden score is lower than a specialty mattress should be applied. At‑risk care plans should be placed for all residents because everyone is nds by Unit Managers/charge nurses and wound nurse ensure residents are positioned appropriately, T&P q 2 hours and PRN in bed and wheelchairs/Geri‑chairs, heels elevated, and no skin‑to‑skin contact. - Consult with the nursing team concerning assessment evaluations and assist in planning the skin care treatment.
