Overview
Summary:
The primary objective of the Patient Care Coordinator is to proactively manage patient flow while assuring effective, quality care. This is a highly visible position within the organization that requires exceptional communication and organizational skills. In partnership with leadership, the Patient Care Coordinator supports the assigned patient care area(s) and facilitates patient flow and a systematic flow and patient care. Duties include leading shift huddles, completing patient assignments, supporting daily operations, assuring team members meals, first-line resolution of patient/team members/provider complaints, elevating appropriate items to the Nurse Manager/Director of Nursing.
Additionally, the Patient Care Coordinators support organizational quality initiatives and throughput goals as well as completion of required daily reports and checklists.
- Acts as a clinical and professional role model within department
- Mentors team members in evidence based clinical practice, effective communication, and teamwork
- Provides oversight to achievement of departmental quality metrics
- Provides real time feedback on opportunities within practice
- Daily Huddles
- Utilizes weekly huddle list to brief team members
- Provides updates on current departmental conditions
- Receives and addresses feedback from team members
- Facilitates daily team member/safety huddles in absence of Nurse Manager
- Focuses on patient safety and ensures immediate follow up
- Completes daily assignment sheet
- Assign team to appropriate care areas based on team competences and NC board of nursing
- Assures regular rotation of assignments
- Assures communication tool/phones are signed-out and logged appropriately
- Correctly logs sick calls and no shows
- Team Member Engagement
- Identifies and addresses short-term team member needs
- Communicates needs effectively with Nurse Manager, staffing resource center, house supervisor and ancillary leaders as appropriate.
- Participates in team member and patient rounds as assigned by manager
- Assures departmental readiness for accreditation
- Validates departmental cleanliness and organization
- Assures daily/weekly checklists are completed (crash cart, eyewash, etc.)
- Facilitates departmental throughput
- Manages capacity at departmental level by monitoring flow and addressing concerns
- Utilizes effective communication to address barriers and challenges
- Identifies delays in patient disposition and addresses with nurse and/or provider
- Works to expedite all patients to patient care area
- Collaborates with transfer center and ancillary departments to remove barriers and facilitate timely and seamless reception/transfer of patients.
Facilitates team member meal breaks
QualificationsRequired:
- Current North Carolina RN licensure (or compact state)
- Minimum two (2) years patient care experience
Preferred:
- Bachelor of Nursing
- Specialty Certification
- Experience working as a charge nurse in an acute care setting
