Horizons Residential Care Center is seeking an experienced Registered Nurse who can lead a clinical team, recognize changing resident needs, and follow through on the details that make care safe and consistent.
Our nonprofit organization serves children and adults with intellectual and developmental disabilities, including individuals with complex medical needs. As Nurse Unit Manager, you will help build a supportive environment where residents are treated with dignity and staff have the guidance, training, and accountability they need to succeed.
Reporting to the Director of Nursing, you will oversee the daily nursing operations of your assigned unit, coordinate resident care, and provide hands-on clinical support when needed.
What Success Looks LikeYou are visible and approachable on the unit. You address concerns promptly, communicate expectations clearly, and follow up until issues are resolved. You support your team while holding staff accountable for safe care, accurate documentation, and professional conduct.
What You’ll DoOversee daily nursing care and respond promptly to changes in resident condition.
Coordinate admissions, hospital returns, transfers, and follow-up care.
Ensure physician orders, medications, treatments, and nursing care plans are implemented accurately and on time.
Monitor care involving seizure disorders, feeding tubes, tracheostomies, and other complex medical needs.
Provide direct nursing care and assist with coverage as needed.
Support and develop staffCoordinate nursing assignments and coverage based on resident needs and staff competency.
Provide clear direction, practical coaching, and timely feedback to nursing staff.
Partner with direct care leadership to reinforce resident care expectations and address concerns.
Help orient new staff, assess competencies, and identify additional training needs.
Address performance concerns consistently and collaborate with the Director of Nursing on corrective action and evaluations.
Maintain quality and accountabilityReview medication and treatment administration records, nursing notes, incident reports, and other clinical documentation.
Conduct routine chart audits and follow through on identified gaps.
Review incidents, medication concerns, falls, seizures, infections, and hospital transfers to identify trends and opportunities for improvement.
Reinforce infection prevention, resident rights, safety practices, and regulatory requirements.
Maintain ongoing survey readiness and participate in quality improvement activities.
Communicate and coordinateKeep the Director of Nursing informed of significant clinical concerns, staffing needs, and unresolved issues.
Communicate professionally with families, guardians, providers, and interdisciplinary team members.
Ensure required notifications and follow-up are completed and documented.
Contribute to care conferences, resident care planning, and emergency preparedness.
Required QualificationsCurrent, unrestricted LPN or RN license authorizing practice in North Carolina.
At least two years of nursing experience.
Experience caring for individuals with intellectual/developmental disabilities or patients with complex medical needs.
Strong clinical judgment and the ability to recognize, prioritize, and elevate concerns.
Ability to coach staff, address difficult conversations professionally, and follow through on expectations.
Working knowledge of medication and treatment documentation, infection prevention, and clinical recordkeeping.
Strong organization, communication, and electronic documentation skills.
Preferred QualificationsThree or more years of nursing experience.
Charge nurse, nurse supervisor, or other nursing leadership experience.
Experience in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), pediatric care, long-term care, or medically complex residential setting.
Experience with feeding tubes, tracheostomy care, and seizure management.
Experience with chart audits, survey preparation, staff competency assessments, or quality improvement.
Familiarity with Therap or similar electronic documentation systems.
