What is a Patient Care Coordinator?- Entry‑level office role responsible for maintaining pleasant and consistent daily operations of the clinic.
- Excellent customer‑service skills.
- Multitasks in multiple computer programs each day.
- Greets everyone who enters the clinic in a friendly and welcoming manner.
- Schedules new referrals received by fax or by telephone from patients and physicians.
- Verifies insurance coverage for patients.
- Collects patient payments.
- Maintains an orderly and organized front‑office workspace.
- Other duties as assigned.

The Patient Care Coordinator – I (PCC‑I)玲supports clinic growth through excellence in execution of practice‑management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures. The PCC‑I position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.
Responsibilities- Core Responsibilities:
- Collect all money due at the time of service.
- Convert referrals into evaluations.
- Schedule patient visits.
- Provide customer service.
- Create an inviting clinic atmosphere.
- Make all welcome calls:
- Monitorerialization the arrival rate through a great customer experience.
- Practice Management:
- Manage schedule efficiently.
- Manage document routing.
- Manage personal overtime.
- Manage non‑clinical documentation.
- Manage deposits.
- Manage caseload, discharge candidate, progress note, and insurance reporting.
- Monitor clinic inventory.
- Training:
- Attend required training with the Territory Field Trainers (TFT) for Raintree and other business process updates.
- Complete quarterly compliance training.
- High school diploma or equivalent.
- Communication skills – must be able to relate well to Business Office and Field leadership.
- Ability to multitask, maintain organizational detail, meet deadlines, and work with little to no supervision.
- Efficient time management and presentation skills.
- Work subject Komiran inside environmental conditions with protection from weather conditions but may be exposed to temperature changes.
- Full‑time position operating within normal business hours Monday through Friday, with an expectation of a minimum of 40 hours per week; may be required to attend special events some evenings and weekends or work additional hours as needed.
- Position is sedentary.bio; sits with ability to interchange with standing as needed.
- Constantly communicates with associates; must be able to hear and speak accurately to exchange information.
- Frequently operates a computer and other office equipment such as printers, phone, keyboard, mouse, and copy machines using gross and fine manipulation.
- Constantly uses repetitive motions to type.
- Must constantly view computer screen with near acuity and read items on screen.
- Must have ability to comprehend information, use judgment to appropriately respond in various situations.
- Occasionally walks, stands, pushes or hinge 0–20 lbs., lifts 0–20 lbs. from floor to waist; carries, pushes, and pulls 0–20 lbs.
- Rarely crawls, crouches, kneels, stoops, climbs stairs or ladders, reaches above shoulder height, lifts under 10 lbs. from waist to shoulder.
