Medical Social Worker – Home Health (Part‑Time/PRN)

MSA Home Health-Coastal, part of Medical Services of America, seeks an experienced Part‑Time/PRN Home Health Social Worker for its North Charleston location (Charleston, Dorchester, Berkeley), SC.

Pay range
: $32.00/hr - $34.00/hr (based on skills and experience).

Location: North Charleston, SC (Charleston, Dorchester, Berkeley counties).

• Provide services in accordance with the physician’s plan of treatment and assist the physician and other members of the home health team to understand the significance of social and emotional factors related to the client’s care.
• Work with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Develop, prepare, and maintain individualized client care progress records with accuracy, timeliness, and according to policies.
• Recognize and utilize community and family resources to assist the client in long‑term plans for life in the community or to learn to live within the client’s disability.
• Cooperate with appropriate staff to provide staff education when requested and arranged by the client care manager.

• Bachelor’s or Master’s degree in social work, sociology, psychology, or rehabilitation counseling from an accredited 4‑year college or university.
• Minimum of one year experience in social work, preferably in a home care agency, hospital, rehabilitation center or mental‑health program.
• Currently licensed to practice as a social worker in the state of South Carolina.
• Valid driver’s license.
