Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.
Schedule: Monday - Friday;
Call Required
Territory: North Charleston
In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives the best care possible—care grounded in compassion, clinical excellence, and connection.

Responsibilities
- Empower Through Engagement:
Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch. - Collaborative Care Champion:
Partner closely with our Clinical Director to assess the eligibility and suitability of clients for hospice services. - Nurturing Direct Care:
Serve as a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount. - Nursing with Heart:
Deliver top‑notch nursing care rooted in proven principles and techniques. - Educator and Advocate:
Empower patients and families through education, encouraging their active participation in creating personalized care plans. - Call of Compassion:
Share responsibility of call duties, being there when it matters most.
Qualifications
- Current, unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical‑surgical/acute care setting. Hospice experience a plus.
- Computer proficient in typing and various programs, including EMR background.
- Valid CPR certification if required by state.
- Current driver’s license, vehicle insurance, and dependable transportation.
Pay: $70,000 - $80,000 per year (average). Salary will be based on experience, qualifications, and responsibilities, confirmed at the interview conclusion.
