As a hospice and palliative care nurse practitioner, you will provide diagnosis, treatment, and care under physician supervision, focusing on palliative care and supporting families along their healthcare journeys.

• Provide diagnosis, treatment, and care under physician supervision, focusing on palliative care.
• Assist families along their health care journeys with compassion and support.
• Participate in interdisciplinary team meetings and care conferences.
• Use professional development opportunities to advance practice.

• A heart to serve patients and families with passion.
• Education: Graduate of an accredited master’s, post‑master’s, or doctoral nursing program with formal NP preparation.
• Licensure: Current unencumbered APRN licensure as a Nurse Practitioner, with full DEA privileges and national certification in the NP specialty.
• Experience: At least 2 years within chronic disease management/long‑term care in the last 5 years. Hospice and/or palliative care background preferred.
• Required: Reliable transportation; able to sit, stand, bend, move intermittently and lift 80‑100 lbs.
