Responsibilities- Provide service to patients in designated care settings, including acute, short and long-term care, by assisting physicians, assessing patients, and treating injuries and ailments.
- Obtain thorough medical history from patients and perform examinations/assessments to determine patient needs.
- Collaborate with other healthcare professionals to analyze and interpret information to develop and establish diagnoses.
- Coordinate care and navigation by rounding on patients when necessary.
- Work in collaboration with providers and independently to provide care; formulate treatment plans and prescribe medications based on efficacy, safety, and cost as legally authorized if necessary.
- Maintain complete and detailed records of patient health care plans and prognoses in a timely manner.
- Consult with or refer patients to appropriate specialists when conditions exceed scope of practice or expertise.
- Provide patients with information to promote health, reduce risk factors, or prevent disease or disability; counsel patients and families on self-management and treatment plans tailored to individual circumstances.
- May cover hospital consultations, rounding, and assist with patient procedures as needed.
- Engage in research within area of specialty and apply findings to improve patient care and outcomes; incorporate evidence-based practice guidelines.
- Maintain current knowledge of state regulations for advanced practice provider practices and payer systems (e.g., Medicare, Medicaid, managed care, private payers).
- Demonstrate extensive advanced knowledge of medical principles, practices and techniques.
- Completion of a master’s, post-master’s, or doctorate from an accredited nurse practitioner program (CCNE or NLNAC).
- Current licensure by the applicable state board of nursing for advanced practice; current DEA permit to prescribe controlled substances.
- Certified Family Nurse Practitioner (NP-C) or (FNP-BC).
- Demonstrated competence and provision of care with adequate patient volume in the past twelve months, or recent completion of master’s/post-master’s degree program; experience must correlate to requested privileges.
- NP or PA position;
Schedule:
M-F, no holidays, nursing home call;
Clinic only; new grads considered. - Opportunities to perform minor procedures; team includes 1 physician & 10 APPs; FTE preferred at 1.0, not less than 0.8.
Currently licensed and able to practice in the state; maintain required department competencies and certifications. DEA registration and ability to prescribe controlled substances as applicable.
