12-hour. Days; 6:00 AM - 6:30 PM; every other weekend and holiday.

Requirements

BLS Certification
Licensure:
Active LPN license required
Experience with Dementia, Parkinson's, and MDD patients preferred
Proficiency in Electronic Medical Records (EMR) systems, specifically Point Click Care (PCC)

Responsibilities

Provide direct patient care including assessment, planning, implementation, and evaluation
Administer medications and treatments as prescribed
Monitor patients' conditions and report any changes to the nursing team
Assist with patient education and support family involvement in care
Maintain accurate and timely documentation in EMR
