Weekend Nurse Practitioner (Skilled Nursing)
Aviata Health Group is looking for a skilled Nurse Practitioner with skilled‑nursing‑facility (SNF) experience to join our family. If you thrive in a collaborative team environment and want to make a difference in residents’ lives while advancing your care skills, we’d love to talk.
Responsibilities- Serve as the leading internal medicine provider for the SNF on Saturdays and Sundays.
- Collaborate closely with the interdisciplinary team and physicians to reduce weekend hospitalizations.
- Provide care to the entire resident population, including short‑term and long‑term stays, under the guidance of medical directors and attending physicians.
- Conduct new‑admission evaluations and coordinate discharge visits.
- Address patients exhibiting changes in condition.
- Offer advanced care planning, comprehensive health assessments, differential diagnoses, and prescription management within the SNF protocol.
- Prescribe medications and treatments, perform comprehensive physical examinations, and develop patient‑management policies.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate effectively with attending physicians and specialists regarding patient care.
- Healthy work‑life balance with flexible scheduling.
- Malpractice insurance and liability coverage up to $1 MM/$3 MM provided by the company.
- Revenue‑split compensation structure.
- Advanced Practice Licensure (APN) with a Florida license.
- Active license and DEA number.
- Prior experience in nursing or related medical fields.
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast‑paced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience emphasizing teamwork within a healthcare setting.
- Master’s degree required.
