You will assess and manage
** acute and chronic medical conditions**, develop individualized treatment plans, provide ongoing patient follow-up, and collaborate with physicians, facility staff, specialists, and interdisciplinary care teams.###

Key Responsibilities
* Provide comprehensive

** to residents within skilled nursing facilities.

** Qualifications
* ** Active Nurse Practitioner (NP) license in CT.
*** Minimum
** 2 years of post-certification clinical experience preferred**.
* Experience in
** skilled nursing facilities, primary care, hospitalist medicine, emergency departments, or intensive care
** preferred.
* Strong clinical assessment, communication, and organizational skills.
* Comfortable managing
** acute and chronic medical conditions
** in an older adult population.
* Ability to work independently within scope of practice and collaboratively with an interdisciplinary care team.
* Proficiency with
** EHRs and clinical technology**.
