RN Case Manager

Seeking a registered nurse to deliver quality healthcare outcomes through safe, individualized discharge planning and timely utilization management services. The role involves proactive collaboration with patients, families, and healthcare teams to optimize reimbursement and patient care.

• Establish safe discharge plans tailored to patient needs
• Coordinate utilization review and management activities
• Collaborate effectively with healthcare teams and patients
• Ensure compliance with clinical and social needs post-discharge

• Graduate of an accredited nursing program
• Minimum of three years clinical experience; two years in utilization review, management, or case management preferred
• Knowledge of discharge factors, community resources, and social/physical health influences
• Proficiency in computer applications such as word processing and data management
• Current Registered Nurse license in the state
• Ability to provide age-appropriate care and service
