Case Manager (Service Coordinator)
Service Coordination Inc® (SCI) is seeking dedicated Service Coordinators who are passionate about serving and empowering individuals in the community. The role involves comprehensive case management, person‑centered planning, and coordination of services for clients with intellectual or developmental disabilities.
Position SummaryProvider of case management services to individuals who have an intellectual disability and/or developmental disability. Primary responsibilities include establishing positive relationships, developing and monitoring annual Person‑Centered Plans (PCPs), ensuring state‑mandated requirements are met, and advocating for client services.
Key Responsibilities- Advocate on behalf of individuals, provide resource information, and educate them for self‑determination.
- Conduct comprehensive assessments (CA) and complete Person‑Centered Plans (PCPs).
- Coordinate and facilitate services, including referrals to DDA and other agencies.
- Meet with individuals, families, and support staff to gather information and document eligibility.
- Prepare and submit necessary paperwork, waivers, and recertification forms (e.g., RECON, AIR).
- Travel to client locations, support staff, and community resources as needed.
- Maintain confidentiality, HIPAA compliance, and accurate documentation.
- Engage in ongoing professional development and participate in leadership meetings.

- Bachelor's degree in a human services field from an accredited program.
- Or, a bachelor’s degree in a non‑human services field, and at least two years of experience in a human services setting.
Experience working with individuals who have developmental disabilities.
Physical DemandsOffice‑based position, more than 95% stationary; ability to operate a computer, keyboard, mouse, and other office equipment; occasional lifting of items weighing 10–25 pounds; regular interaction with others; ability to drive a motor vehicle.

Compensation & Benefits- Competitive base salary: $64,395–$79,861, based on education, experience, and certifications.
