The community nurse will work as part of an integrated team in North Lincolnshire, contributing to family health needs assessment. Working with families, children and young people aged 5‑19 (including SEND) years, the post holder will partner with families and other agencies to develop, implement and evaluate packages of care both individually and in groups. The community nurse will also support the wider public health agenda for Children and Young People through individual, group and community programmes of care.

This post offers a great opportunity for a Registered Nurse (RN/RSCN/RNC) with post‑qualification experience. It is an engaging role that allows the successful candidate to further develop nursing skills within a community setting and deliver quality, evidence‑based care. The organisation supports training needs and offers an excellent learning and development programme. Regular safeguarding, management and clinical supervision will be provided, with support from the leadership team.

Responsibilities
  • Work as part of an integrated team in North Lincolnshire and contribute to family health needs assessment.
  • Collaborate with families, children and young people aged 5‑19 (including SEND) to develop, implement and evaluate individual and group care packages.
  • Support the wider public health agenda for Children and Young People through individual, group and community programmes of care.
  • Provide safeguarding, management and clinical supervision, liaising with the leadership team.
  • Work flexibly and respond to changing demand in a delegated workload.
  • Supervise others and facilitate learning in one‑to‑one and group settings.
  • Work independently as an autonomous practitioner, making clinical judgements, decisions and taking responsibility and accountability for chosen actions.
  • Risk‑assess and engage in lone work within the community, organising drop‑in sessions in schools or educational settings.
Qualifications
  • NMC registration as Registered Nurse or Midwife.
  • Evidence of post‑registration training.
  • Evidence of recent professional development.
  • Experience of undertaking holistic health assessments.
  • Experience working with children, young people and their families.
  • Demonstrated understanding of the Healthy Child Programme and the role of the School Nurse.
  • Knowledge of current public health and health promotion issues relevant to community children’s services.
  • Understanding of child protection and safeguarding policies and procedures.
  • Ability to demonstrate individual responsibility in relation to health and safety.
  • Ability to recognise and understand equality and diversity issues.
  • Proficiency in software packages appropriate to clinical care.
  • Ability to supervise others.
  • Sound knowledge and application of up‑to‑date evidence‑based clinical trends and practice.
  • Good interpersonal skills and ability to develop and maintain positive working relationships.
  • IT literacy, including word processing, email and internet use.

  • Capacity to work autonomously and make independent clinical judgements.
  • Capability to facilitate learning in one‑to‑one and group settings.
  • Competence to lone work and risk‑assess in the community.
