We have an opening for a Bilingual Analyst, Case Management position with Aetna’s Long‑Term Services and Supports (LTSS) team. This is a field‑based role based out of North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler, FL, requiring 50‑75% travel to meet with members face to face.
Schedule:
Monday‑Friday, 8:00 am‑5:00 pm, standard business hours.
Case Management Coordinator is responsible for telephonically and/or face‑to‑face assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate overall wellness. The coordinator will manage a caseload that includes supportive and medically complex members, develop proactive action plans to address presented issues, and enhance short and long‑term outcomes. Duties include determining appropriate services and supports, such as prior authorizations, coordination with PCPs and skilled providers, condition management, medication review, and community resources.
Required Qualifications- Bi‑lingual in English and Spanish.
- 1+ year of experience in behavioral health or a long‑term care setting.
- Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler, FL.
- Reliable transportation and willingness to travel up to 50‑75% of the time; mileage reimbursed per company policy.
- Ability to multitask, prioritize, and adapt to a fast‑paced environment.
- Effective written and verbal communication skills.
- Managed care experience.
- Computer proficiency in Microsoft Word, Excel, and Outlook; case management and discharge planning experience.
- Bachelor’s degree required, preferably in Social Work or related field.
- Master’s degree preferred.

Typical base hourly rate or annual salary: $21.10 – $36.78. Eligible for CVS Health bonus, commission, or short‑term incentive programs.
