Key Responsibilities **
* ** This is a float position responsible for delivering patient care in all Physician Practices through the nursing process of assessment, planning, implementation, and evaluation.**
* ** Greets and escorts patients through the medical practice.**
* ** Completes nursing assessments of patient by completing history of present disease and illnesses with accuracy and documents appropriately in patient electronic medical record.**
* ** Collaborates with physicians and other health team members in implementing physician orders.**
* ** Incorporates holistic approach to Patient Centered Medical Home, thus moving beyond problem focused care.**
* ** Implements and documents clinical and technical aspects of care.**
* ** Evaluates patient’s response and understanding to plan of care by using teach back method.**
* ** Openly and appropriately communicates with patient, physicians and other members of health team.**
* ** Serves as a liaison for patient with outside agencies, businesses and providers, establishing a therapeutic relationship with the patient and family.**
* ** Respect patient’s rights to privacy, dignity and confidentiality.**
* ** Maintains positive interpersonal relationships with all internal and external customers.**
* ** Maintains established health system and work area policies and procedures, objectives, guest relations, safety, environmental and infection control guidelines**
* ** Performs other duties as assigned.
**** Requirements**
* ** Clinical skills in delivering patient care in an outpatient setting.**
* ** Obtains and maintains successful completion of BCLS Healthcare Provider Course.**
* ** Communications effectively with physicians, patients and coworkers to maintain respect and a positive work environment.
**** Education/Certifications**
* ** Graduate of an accredited school of nursing.**
* ** Licensed to practice professional nursing in the State of Ohio
**** Experience**
* ** Previous experience in medical practice or outpatient clinic preferred.*
