Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team.
Three 12 hour shifts with alternating weekends - days to be determined. 8pm - 8:30am. This position covers our 4 county service area including Manatee, Sarasota, Charlotte and DeSoto.
Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.
Primary tasks, duties, and responsibilities:- Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
- In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
- Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
- Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
