Anticoagulant Nurse Specialist

Applications are invited for an enthusiastic Clinical Nurse Specialist to work within the Anticoagulant team within Northumbria Healthcare NHS Foundation Trust. The focus of this role is to participate in the provision of a safe and coordinated anticoagulant service across North Tyneside and Northumberland. You will be part of a team of nurses and support staff undertaking anticoagulant monitoring for the introduction and maintenance of patients on anticoagulation, providing education, advice and support to outpatients and inpatients.

Acting as a resource to other professionals across primary and secondary care is a key element of this post.

Main duties of the job
  • Undertake INR monitoring clinics within the hospital and community setting, seeing both new and follow‑up patients.
  • Initiate anticoagulation clinics.
  • Monitor in‑patients on anticoagulants in all wards.
  • Advise and educate patients and staff on all anticoagulants.
Job responsibilities

The anticoagulant service at Northumbria Healthcare provides care for approximately 1,800 patients, with approximately 26,000 attendances per annum.

  • Provide a safe, effective and coordinated anticoagulant service for North Tyneside and Northumberland in community and acute sectors.
  • Offer an accessible specialist anticoagulant advice service for primary and secondary care, including advice on diagnosis and management of the prevention of venous thromboembolism.
  • Develop and provide educational support for all grades of staff from a variety of professions, as well as for patients and their carers.
  • Deliver highly specialist clinical assessment, planning, implementation and evaluation for a large number of patients receiving anticoagulation, including initial assessment, completion and interpretation of tests, and education of patients newly commenced on anticoagulation.
  • Assess individuals' suitability for anticoagulation and signs of underlying disease requiring referral to other clinicians.
  • Provide medication to patients without a medical prescription through patient group directions.
  • Give highly specialist advice concerning all aspects of anticoagulant care and VTE diagnosis and management to clients, carers and all staff within primary and secondary care up to consultant/GP level.
  • Advise on warfarin doses and manage anticoagulation in situations where it may be unstable or needs adjustment, such as pre‑ and post‑operative periods, drug concordance issues, drug interactions, concurrent illness and lifestyle factors.
  • Manage patients who are significantly under‑ or over‑anticoagulated, assessing haemorrhage risk and arranging warfarin reversal for dangerously over‑anticoagulated patients.
  • Ensure accurate and up‑to‑date records are maintained.
  • Perform physical skills including venepuncture, capillary and point‑of‑care blood testing, and administration of subcutaneous injections.
Person Specification
  • Registered Nurse at 1st Level.
  • Educated to degree level or equivalent.
  • ENB 988 Teaching & Assessing in Clinical Practice or equivalent experience.
  • Mentorship Qualification or relevant equivalent qualification (essential requirement for the post; expectation that this will be completed within an 18‑month period).
  • Supervisory/Management Qualifications.
  • Experience managing and conducting outpatient services, including counselling and communicating with patients.
  • Research and audit experience.
  • Previous experience managing patients on anticoagulant medication.
