The Advanced Nurse Practitioner will work as an autonomous, highly skilled clinician within Collingwood Health Group, providing advanced clinical assessment, diagnosis, and management of patients presenting with acute and undifferentiated conditions. The post holder will deliver high-quality, patient-centred care through same-day and urgent appointments, including the management of minor illness, minor injury, and acute exacerbations of chronic disease. Working within their scope of practice and professional regulatory framework, the Nurse Practitioner will independently prescribe, request and interpret investigations, initiate treatment plans, and ensure appropriate follow-up or escalation of care where required.
The role requires strong clinical decision-making, effective risk management, and the ability to work both autonomously and collaboratively within a multidisciplinary primary care team. The post holder will contribute to service development, clinical governance, and continuous quality improvement, while maintaining accurate clinical records and upholding the highest standards of professional practice, confidentiality, and patient safety. The Nurse Practitioner will support health promotion, patient education, and preventative care initiatives, contributing to improved patient outcomes and enhanced access to timely primary care services.
You will work with the team to develop new and innovative concepts, models, methods and practices to deliver Primary Care.

We are based in North Shields in the North East of England, at the coast, 8 miles from Newcastle. Collingwood Health Group is the largest practice in North Tyneside with a patient list of approximately 24,500 over 4 sites.
We are a well established, forward thinking practice providing care to diverse communities. Our scale and multi-site presence enable us to offer a dynamic and resilient approach to service delivery. Our multidisciplinary team is led by 9 GP partners, and supported by a wide ranging multidisciplinary team that includes, Salaried GPs, Advanced Nurse Practitioners, Advanced Care Practitioners, Physician Associate, Practices Nurses, Nurse Associate and Healthcare Assistants.
We are also fortunate to have an integrated pharmacy team comprising of clinical pharmacists and pharmacy technicians who contribute significantly to the safe and effective management of prescribing and medicine use. We are a training and teaching practice. We support learning and CPD through regular clinical meetings and encourage a healthy work/ life balance.
