You will manage a caseload, conduct monitoring and wellbeing visits, and review arrangements to keep people safe, positive and person-centred.
The role requires experience with vulnerable adults, a Level 3 Health and Social Care qualification (or equivalent), knowledge of safeguarding and the Mental Capacity Act, and a valid driving
