We are making some changes here within our specialist Social Work Team, with the introduction of an Initial Response Hub to ensure we enhance the way we respond to our residents and their families / carers.
We support people whose lives are impacted by their mental health or by addictions to stay safe and live as independently as possible. The team works closely with colleagues from Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust and North Tyneside Recovery Partnership (NTRP) to provide a holistic, multi-disciplinary approach to adults experiencing severe and enduring mental health issues and / or those who use substances.
The vision of the team is to work in collaboration with individuals to support them to achieve the best possible outcomes, to promote their safety and well‑being and to empower individuals to be as independent as they possibly can be. There is a strong commitment to working in partnership with residents, carers, professionals and providers, keeping the individual at the centre of the process throughout.
The Initial Response Coordinator is a newly created post. The role involves conducting initial assessments, offering advice and guidance, ensuring timely referrals to appropriate services, coordinating care pathways, collaborating with multidisciplinary teams, stabilising crises, reducing harm and promoting recovery while maintaining a person‑centred and trauma‑informed approach. The post holder will also line‑manage Mental Health Reablement Workers and work collaboratively with a range of professionals, including Social Workers, Housing Officers, Police, and other relevant agencies.
The Initial Response Coordinator will be supported by a Team Manager, Assistant Team Manager, Advanced Practitioner, Social Workers and Mental Health Reablement Workers and would be joining at an exciting time which can make a real difference to how we can offer support.

Salary Scale – Grade 8 £ 34,434 – £38,220 pa.
